The purpose of this project is to extend an existing website with a registration form.

Implementation needs to be done in php-generated valid xhtml.
The link "programs" shows a schedule of upcoming events. Next to each event a "register" button should be placed. That button takes the user to a new page, which contains the registration form. That page also shows which event has been selected, with an option to change to another event (for example, in a select box). The form fields to be implemented are the following:

Followed by these form fields:

## Deliverables
Attached to this project, you will find a zip file. In that zip file there are two folders:
* contact: contains the [login to view URL] file for the current 'contact' page.
* programs: contains the [login to view URL] file for the current 'programs' page.
as you will see in the [login to view URL] file for the contact page, an array is used to define field properties. This array also holds the field names for both the english and dutch versions. Please use the same structure for the register page, such that we can make the registration form bilangual.
As the form needs to be signed, it cannot be submitted and sent by email. So a "print" button will have to be made. The website allready uses a seperate css file for printing, which you may add to for layout out this form on print.
The address shown in the current 'contact' page in the left column, should also be in the left column of the new 'register' page.
Layout of the register page done by css is preferred, but in a html table is acceptable. Please say in your bid how you will implement layout; css or table.
Please lay out the form fields sensibly; don't place them all under each other
**Deliverable: a new [login to view URL] file in a new /register/ directory containing the registration page, the changed [login to view URL] file for the /programs/ page and if appropriate the altered [login to view URL] file**
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
