Webmail Registration & Login Form

I am looking for someone to build me a form that I can incorporate into my website to allow users to register an email account on my webserver. I have not decided on a type of mail server or an operating system and am completely happy to work around the coder. It should be noted that I can use both UNIX and Windows operating systems for my mail server and that I can run any mail server package (Surge Mail, Windows Mail Server etc). The user should only be asked for username, password, confirm password & captcha on registration. Once a user has registered that they should be asked to login using a login page and then taken to their inbox where they can send / receive emails etc. Please only bid if you know what you are doing / are a decent web designer and will use proper coding techniques.
