Need a Developer with php, mysql skills

Hi Developers, I need three webpages to be created. For this I require a PHP developer Brief Details: Listing Page ? List of rows with a few columns which are sortable and also has one or more searchable fields via search box. Add Page ? basic form to add new record. Form will be around 40 fields or less Edit Page ? basic form to edit record ? Form will be exact as above page so hopefully can just be one page combined. Both add/edit pages should have standard validation also.(ie email, phone, fax, required fields, etc) These pages should be in PHP with a mySql backend. I currently use the CSS Bootstrap from Twitter but if you have a different CSS framework in mind please let me know because I would like a better look than I have right now. The design does not need to be complex. Simple and clean and up-to-date is good. Now from your side I need to know some details which are: Number of Hours it will take you to complete>>>> Your Hourly Rate>>>>> Can you complete it within 48 to 72 Hours >>>>>>>>>>.
