I need two data entry forms created.
They are going to be re-worked to go with another site so the visual layout (beyond what is required to verify that the project works) is not extremely important for this project.
The first form is a single entry form which has a form where the data is filled out and submitted.
The second form requires the user to upload an CSV file then has them map the required fields to fields in the uploaded CSV file.
The single request entry web form will be a modified version of
our catalog request page on the website. Visitors will be able to fill
out their information and press submit and the information will go
into the Leads Table. The single request entry
web form will have the following enter the following fields:
First Name - Required - Text Box
Last Name- Required - Text Box
Company- Not Required - Text Box
Address 1- Required - Text Box
Address 2- Not Required - Text Box
City- Required - Text Box
State- Required - Drop Down Menu
Zip/Postal Code- Required - Text Box
Country- Required - Drop Down Menu (USA default)
Phone - Not Required - Text Box
Email - Not Required - Text Box
Involvement - Required - Drop Down Menu
Literature Requested - Required - Drop Down Menu (Catalog default)
Source - Required - Drop Down Menu
In addition there should be a variable "$csr". If this variable is set to true then this entry form will have a version for the CSRs
to use as well that will have the following fields available to edit:
Rep Name, Date Printed, Means of Delivery.
The second form is the bulk import entry web form through which the user will be able to enter multiple
entries into the Leads Table. The user will be able to
upload a csv file and choose which columns go to which field by using
drop down menus. The Bulk Entry Form will populate the Leads Table.
