Upload 142 columns of data to an existing MySQL database in Joomla CMS -- 2

A MySQL database has been created in a Joomla CMS which has 75 columns and 1288 rows of data (See attached spreadsheet). An additional 67 fields are required to be created and then the data is to uploaded to the corresponding fields in the mysql database.
All the data from the enquiry from is to be included in database which is part of the 142 fields.
