SQL Quote Form

I currently run 7 websites that offer promotional products to consumers. Each website has a quote form that the customer must complete. When the customer completes the form, it generates a sequential confirmation number and emails the quote information to the office. We are currently using a free program called form tools running on a windows server and mysql. I am looking to upgrade the quote form and move away from FormTools. The features that I am looking for is: Basic data collection html form (currently designed and in place) Ability for file upload (must be graphical and not allow customer to submit until file completely uploads) Generate sequential quote number and display confirmation form to customer. Send customer a confirmation email Send backoffice a email containing all data the customer submitted Offer a back-office website that shows quotes for all 7 sites in real time with ability to click them to see full details. Each form must include a captcha to prevent spam. I am designer by trade, but very consumed with other projects. This said, I do know this is NOT a complicated tasks, and any experienced designer could design this in one full day/night. We need this system designed ASAP. There is a budget set for this project, it is $125.00 USD PER SITE - TOTAL $875.00 I have used this site before and was very pleased with the quality of providers.
