Wordpress plugin including form

I need a wordpress plugin, which will add a link and a QR code to existing posts. The process will begin with clicking "Add a "Wandering" activity to a post" This will have the following functions: 1. On pressing, it will open a form which the user will fill out (form also needs to be created) and submit. detailing of the form will be sent to relevant bidders. 2. The editing to the post will be adding a QR code and a link in the end of the post, which will be retrieved on submission of the form. Notes: When installing the plugin, name and email will be taken from the user, and saved for use in form submission. I have a much more complex form which can be edited and shortened to give the form in question, which also has auto location and QR code creation already implemented in it.
