We need to simple PHP form to forward certain information provided by the user. Please see the attached screenshot. Currently, when the user presses the button labeled "Click Here to Request a Free Detailed Quote", the user's mail client opens a draft email with the information on the form. We would like to change this so that this button instead links to another form on our site. The information from the first form should be passed to the new form-- the user should be able to provide some new information, and then the .php form should send an email to us with the user's request, and a confirmation email to the user. We will provide a .php file containing our current contact form to the winning bidder. This form already provides most of this functionality-- it would need to be adapted to accept the information being passed to it from the first form, and also to validate the new information (using the existing validation process) prior to sending the emails. The final products should validate as proper XHTML/CSS using the links in the footer of our existing page template. PLEASE NOTE: Our hosting provider runs PHP version 4.3.11; any solution must runon this version of PHP.
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
