Design 12 html Sales Docs- Requires Financial / Mortgage knowledge - design + math

The task is to design and provide the math equations so they can be coded up. [login to view URL] is pretty much done and an example of what I expect. It must be done in html, must print nicely on one page, and any lines have to be done with tables - this is so one doc can be emailed, printed, efaxed, or viewed with a browser. The information will be posted from lead.php. Step1 is additional questions on [login to view URL] (see example in source bin), pertaining to that exact doc. Step 2 is choosing a delivery method, and step 3 is generating the actual document. For now, I only need you to produce a nice looking document, with the math equations to get the numbers. The vast majority of clients will print these out so they should look good on a variety of printers and in both BW and color. if it helps any to understand, this is a product to compete with <[login to view URL]> Code is not required, though if you're a team or small company and can deliver both I'd strongly prefer that. Buyer will receive exclusive and complete copyrights to all work purchased.
