a script to generate quiz with certification

I am looking for a script (PHP is preferred) to create a web based quiz (exam) -single/multiple- and if the user successfully answered the questions, the system will automatically generate a certificate (easy to customize) for him/her. The web based application should have below features: 1) Professional GUI 2) Objective questions, multiple choice, single choice, true-false, fill in the blanks 3) Time bound/ unlimited time quiz 5) User restriction for a quiz Jump to sections during quiz 6) User login - to save exam for later and they can come back to complete the quiz 7) Detailed Admin section can be easily customized with reports 8) Certificates print 9) any other proposed funstions a demo has to be demonstrated before accepting the final project ..
