New patients to a doctors office need to complete a ipad questionaire for basic health information. The receptionist will prepare the app for the patient by using a secure log in on the ipad which will connect to an online sql server backend. This will bring up a list of patients populated by the SQL server (not included in the project). The receptionist will select the correct patient from the list and any available information in the database will prefill the app . The patient will review and correct as necessary.
Information will include first name, last name, middle initial, address, phone number, email address, gender, birthdate, height, weight, dropdown list for reason to see the doctor, dropdown list for how they were referred to the office, list of previous surgery (type of surgery, year, any complications), current medications (name, dose, frequency), allergies to medications (choose "none" or fill list with name of medication and type of allergy), smoking history (yes/no if yes how many a day), a list of about 30 yes/no checkbox questions. Also need an optional page for adding insurance information, and optional page for minors to add contact information of parent/guardian. Place for patient to sign their name with their finger once form is completed. Also want to prompt patient to take a picture of their face for their records using the ipad camera (there should be a outline of a face appear with the camera view superimposed so the patient can take the correct sized picture).
Anticipate this will be a multipage questionaire with back and forth navigation. Some fields like name, address, birthdate will be required and validated prior to uploading. Once completed the patient will hit a "submit" button and the information will be sent to the database (the SQL server update process is not a part of the project) via wifi or via cell phone. Setup of either comminication option will be done by doctors office.
The doctors office needs to be able to upload a logo (image and/or text) for the top of the app so that the app will be 'customized' to each office after deployment.
I will provide the SQL server tables and stored procedures that will be used to initially sign on to the app, select the new patient, and then to insert the new patient data. This will be posted to the Apple store for the end user to download.
