I retype from the image and then proof every line back against it, so what you get mirrors the source: same words, numbers, line breaks, headings and emphasis, tables kept as real tables (Excel where a table clearly belongs, Word otherwise), and nothing silently "corrected". Anything genuinely illegible is marked [unclear] in place instead of guessed, so you always know where to look.
