Check Hindi Translation of Intake Form

We operate "Never Quit Looking", a global database of missing persons, unsolved homicides, and unidentified remains.
One of the ways we develop case files is through an intake form offered via JotForm ([login to view URL]). The platform allows us to offer an English and Standard Hindi translation. The form gathers the victims details (age, hair color, distinguishing features) and details about their case (setting, situation, location, etc) as well as some related information (police contact details, vehicle, suspect).
We have machine-translated the form into Hindi, but we need someone who can check the translation and tell us what needs to be corrected.
The form has several pages so please review them carefully before bidding. The first page of the form is in paragraph format and is likely to have the most issues. The rest of the form should be pretty straightforward for translation (hair colors, eye colors, locations, country names) and is largely single words (e.g., Nightclub) or phrases (e.g., Orthodontic Braces). Hopefully the machine translation worked pretty well on these, but they all need to be double checked.
You can actually use the link to see the form in its entirety before you bid. Just use the language change icon to switch between English and Hindi.
We hope it wouldn't to large of an undertaking. If possible, just a word document or email explaining the changes that need to be made would suffice.
We are willing to pay a little more for a native speaker who does a good job, but please note that we are paying out of pocket since this is a charity project. It's probably a good job for a student or someone looking for a simpler project. This project is not really time sensitive.
