We use a PDF form for our new clients to ask them for information. The one we are currently using needs a bit of an update, and so we have attached the original template/form, the changes we would like illustrated on the original template and below please find a list of the changes we would like done:
On the first page we would like:
*on the very top we would like the telephone number inserted: 0800 689 1667
*Personal Details to read as Personal Details as appears on passport
*Date of Birth to be moved under surname
*Insert Are you a smoker? After Date of birth and before first line of address
*Below Date Moved in, insert Residential status with tickable options which will be: Renting from private landlord, renting from local authority, home owner with mortgage, home owner without mortgage, living with parents, living with friends/relatives
*the section ‘if lived at current address less than 5 years’ with questions first line of address, second line, post code and date moved in should be in a separate thin lined box
On the second page we would like:
*in the employment section swap round occupation and employment status
*remove business telephone number
On the third page we would like:
*to swap round the ‘Do you anticipate any changes…’ along with the ‘if yes please provide…’ with section 4 income questions excluding the notes box. The notes box should remain as is and halved in size.
*the income section self should become App 1 and partner to become App 2
*the income section and the ‘if self-employed’ section need to separated and placed below income
On the fourth page:
*the section that reads ‘Notes: any buy now…’ to be changed to Any notes? Example: Buy now/pay laters, etc.
On the fifth page:
*below Do you have a Residential Mortgage? Yes or No, if No please skip should read: if No please skip section 8, if Yes please complete section 8.
*’is the address same as your current home address’ to become section 8.
*the second set of yes or no can be removed from section 8
*the second set of three boxes in section 10 (first line of address, second line, etc) are to be removed
* the second set of yes or no can be removed from is the property flat
*At the bottom of the page, below remaining term, monthly payment to be added with an empty £ box near it as are the others.
*Near the current value and lender questions, two more question boxes with £ to be added. These are: Service Charge and Ground Rent
*at the very bottom of the page a new question is to be added: Are you looking to increase your borrowing? If yes, How much? And insert an empty £ box near it.
On the sixth page:
*section 8 is to become 9
*the second question is to read: if No skip section 10, if Yes please complete section 10
*Property Details for new purchase is to become section 10
*the second set of three boxes in section 10 (first line of address, second line, etc) are to be removed and the following question is to be added with tickable options: Type of property: First time purchase, Moving home, second home, Buy to let
*the second set of yes or no can be removed from is the property flat
*the sentence that reads ‘if yes, please provide….’ Can be removed
On the seventh page:
*the 9 near additional properties details is to become 11 and the title is to be changed to ‘Do you own any BTLs? If yes , how many BTLS do you own?’ and a small box near the question for a number to be filled in
*the 1) near mortgage/details is to become an a) and 2) to become a b)
*in both sections a) and b) the questions: Monthly mortgage payment, Date purchased and Have you ever lived in the property need to be added
*at the very bottom of the page a question needs to be added: Do you own more BTLs? With a yes or no tick box.
