I need someone to create an online form for data entry that will generate a series of automated reports. I've attached a document that contains more general notes and direction.

1) The data lists are on the Data Lists Tab
2) The data entry part is on the Meetings tab (on the online form, all of the fields should be drop down for each option except Notes). The fields for the drop down are the Data Lists Tab
3) A sample of the report is on the Meeting Report 3 Tab. This is just one of the reports and I am open to the format being changed (just need to be able to be in a good printable format from what is displayed)
