Need HTML forms designed

I need some html/css forms designed. If you can goto this link and design new styles fitting with the themes of these forms: [login to view URL] What I'd like is for you to grab the attached html page which is 800px wide to represent a form and all html tags and stylize it. All artwork will need to be 100% original or royalty free. I'd like there to be a background outside the form, headers, footers, etc. I only need CSS and Images used for deliverable. If you change anything in the sample html file, then of course that too. I'm hoping to get around 30 different styles
