Custom QR Code

I need to get a custom QR code created from existing graphics.

This is, essentially, using Photoshop (or other image editing system) to blend an existing logo into an existing QR code. There is a short tutorial here that outlines the process: [url removed, login to view]

Your quote should provide for 3 possible designs and 1 round of revisions to the existing design. The final product must be high resolution for print quality (1200 dpi) output.
