Basic Facebook application - Required professional

We require a competition facebook app to be designed and developed for us for our facebook page. We want to have the app as a page on our facebook page where users can fill in the form to ENTER our competition. The trick to this app is that we want the data of the user to be entered into a MySQL database that we can export the data from.
We also want to allow users who have entered to GAIN more entries by SHARING the competition with their friends and when THEIR friends enter the competition they automatically gain another entry. And so on, so we market to everyone saying the more friends they have who enter, they will gain more entries.
This application is REALLY urgent. But we also require a professional with commercial facebook application experience.
This app needs to capture the data from their facebook profile as well as answer additional questions in the competition form.
Need the following data:
Name
age
gender
post code ( can ask this on the competition form )
income ( can ask this on the competition as a drop down with a few options
And a few other questions where they use radio button to select yes or no.
We require the app to store data in our database, so we have access to the data. And MUST have the ability for the user who has entered, to share the competition with their friends, and gain more entries when their friends enter. And then when the friend enteres, they share with their friends and it adds another entry point to that user.
