Neuropsychological Report

We are a Neuropsychological clinic based on São Paulo, Brazil.
One of our most important business are Neuropsychological Assessments. Today everything is done using multiple sources of information and the final report is done manually.
We use Excel, word and a copy paste form a PDF export form surveygizmo.
Our objective is to create a platform using simple software as Excel, Access word word that can:
1) Process all anamneses data filled by the patient or his/her parent
2) Produce custom reports based on those information’s;
3) Be capable to store all data
I’ve attached an PPT file to explain better our needs. Moreover there are all documents to give you more details.
Documents Attached:
1) PPT file with a brief explanation of the project
2) .doc file with an example of the final report (in Portuguese, you must have to translate to English using google translator)
3) .xls file with an export form surveygizmo with all patient data
4) PDF file with an example of an anamneses from one patient
5) PDF with our survey (in Portuguese)
This first project is a very small and MVP for a bigger tech project. Even this MVP will be segmented in 2 phases.
The bigger tech project is a software that will be capable to produce those reports and moreover to manage all patient life in our clinic – Treatment data, agenda management, financial management and more.
