How I would build it: - Ingestion from your source systems into a staging layer, then a modelled warehouse. If the sources speak HL7 or FHIR, I would parse to a canonical model rather than loading raw messages, because dashboards built on raw HL7 become unmaintainable fast. - "Real-time" defined honestly with you. True streaming and near-real-time — a few minutes' latency — differ enormously in cost and complexity.
