online quiz

I have a quick project that involves creating an online quiz. It's a series of 5 questions.

1. The reader answers one multiple choice question at a time

2. The next screen provides affirmation or the correct answer and then clicks to the next question.

3. The final question is a survey question where the reader fills in a few text fields or checks radio boxes and the answers are emailed to me.

This needs to be complete by tomorrow.
