While waiting for my other project to postI have a smaller 4 page, input only pages to a database.

First Page, a welcome page that can pull $get info first/lastname

Second Page, Personal info, I have a start on this 90% complete. One question leads to of two pages, If yes then, Add more personal info to the personal info page, after goto questions page, if no then skip to questions page.

Third Page Questions Page, on submit make PDF and save to folder on web server, add info from first 3 pages to a current Data Base.

Forth Page, Thank You Page

Budget for this is $75, I just need the core.
