Interview sheet with Emailing resulting report as PDF

Budget $2-8 USD / heure

Need to develop a responsive design website with two pages

A) Login page with Microsoft Security

B) Page with 15 to 20 interview questions and respective answers (Pick List, Option buttons and/or Text Box so on, so forth)

C) Upon clicking submit button a report with selected answers to be generated and email the same to respective recipients as a pdf file

D) provide a confirmation page upon sending the email with option to go back to interview page or sign out.

E) Publish the website and fix the reported bugs & corrections

All data is expected to be stored in SQL Server database tables. No Hardcoding.
