Delivery Record Program required
This program will need to be run on a Windows based pc and should be accessible across a small network.
There will be three main menu items which will have submenus attached.
Most of this information will be transposed onto the spreadsheet (see attachment) which will become the main platform for data to be entered.
The three main menu items are Patient, Group and Reports.
Clicking on this icon will direct you to two further options. Update Patient Record or New Patient.
New: This will take you to a new patient record form. The fields to be completed are First Name, Surname, Group, Address and additional notes. As soon as a patient is added and saved, their information should be uploaded to the spreadsheet.
Update Patient Record: This will take you to a pre-entered patient record form where all the fields can be [url removed, login to view] addition to those fields will be some additional options.
Option to make patient inactive from a date. The date box could default to 'now' or have a date field with calendar box. If the patient is made inactive, there should be an optional date field to reactivate them. When they are made inactive a cross will appear in their box on the spreadsheet.
Option to delete a patient. In doing so, patient will change to 'Deleted' group. The Deleted group will not appear on the spreadsheet but their details are available for retrieval at a later date should that be necessary.
Clicking on this icon will direct you to two further options. Add or Edit.
One of the groups will be called 'Deleted' and it is this group where all deleted patient's files will go, and they will be removed from spreadsheet.
Add: Add a group to the system which a patient can be a member of. A patient may only belong to one group.
Edit: Enables you to edit list of groups available that patient can belong to. (including ability to delete a group)
This icon will direct the user to a variety of reports.
List of active patients with the ability to filter by group and /or by date (including all dates).
List of inactive patients with the ability to filter by group and /or by date (including all dates).
Ability to get a list of patients (filter by active or inactive) for a given week or a given month.
Ability to be directed to a spreadsheet (see example attached) which will be where all the information is updated to.
On the spreadsheet a delivery will be indicated by a tick, a non-delivery due to a patient being temporarily inactive by a cross.
The spreadsheet should be able to be sorted by group, or alphabetically within a group.
The dates should be preloaded onto the spreadsheet.
The spreadsheet should be able to have information entered directly onto it. This information should then appear on the patient record form (Date with tick for a delivery, date with a cross if inactive for a given week).
The spreadsheet should have the same click, copy, paste and drag options as an excel spreadsheet.
The overall look of the program should be simple and colourful. It should be user friendly and once something is saved it should be available to another user through the network.
Basically, when a patient receipts a delivery there are two ways for us to enter this information. Either through the Patient Record form or directly onto the spreadsheet.
Whenever a date is required a little calendar box should be available to make entering the date easier.
