I need a well-structured, fillable document that clients can complete on a computer yet still print neatly for a physical binder kept with their will. The file should open flawlessly in common office software and save as a locked, fill-in-only PDF so data cannot be overwritten accidentally. The template must guide a layperson through every key detail an executor will search for: • Personal & professional contacts: attorneys, doctors, CPAs, insurance agents, close family members, and any other critical relationships. • Financial assets: account numbers, current balances, institution names, advisor contacts, plus the specific type of each account or holding. • Liabilities: mortgages, loans, credit lines, outstanding tax obligations. • Digital access: user IDs, passwords or “where to find” credentials, two-factor backup codes. • A concise balance sheet that auto-totals assets and liabilities. • Final instructions & notes section for special bequests or guidance. Layout expectations – Logical section tabs or clearly labeled headings so an executor can skim quickly. – Fields that expand as text wraps and reset when cleared. – Simple data validation (e.g., date and currency formats). Delivery package 1. Editable master file (Word or Excel—whichever achieves the best form controls). 2. Locked, fillable PDF exported from the master. 3. Brief usage guide (one page) explaining how to enter data, save, and print. Once the template is complete I will test it with sample data; any glitches in calculations, formatting, or print layout will need a quick revision before final sign-off. I have attached two documents. One is "Your Estate Roadmap" that is a fillable example . The Miura Group Roadmap is provieds the outline of what I want on the documents. Each section is separated by a blue line and it is likely this will be a multi page documents with separate sections. The bottom of each sections has a NOTES box that enables the client to add addtional information as needed. My logo and disclosure will evetnaully need to be added to the document as well. The document must be easy to use, visually appealing and functional.
