petition wordpress plugin

I would like my current petition website converted into a wordpress website plugin. I would at the end, like the contents of my current petitions transfered to the new website plugin. I would like the template to match/be similar to what I currently have. I would like the new site to work like my existing site. I will provide details of my site to interested parties.
