I am requiring a proficient freelancer to develop an online single page order form. At the top of the form, it will show the logos for our two websites. These must also be included in the PDF at the end of the process. The form should be a single online page that can be accessed via a web browser, it will capture the following information :- 1. Surname 2. First Name 3. Email 4. Phone Number 5. 1st Line of Address 6. 2nd Line of Address 7. Town / City 8. County 9.
