We had a project made on freelancer several years ago, which creates from a template, and fills in a PDF form from an online webform. We need two mods made to it. Add an option to a field on the PDF form Remove the strikethrough on the PDF form. Here is the webform, without the frame. It is a standard Appnitro Machforms webform. We use machform extensivley - however the do not offer a very good front end of once the forms are created. we would like a PHPCRUD type experience for the machfrom / appnitro product. Please familiarise yourself with the machform product before quoting for the system. We would like the ability to interface php crud on machforms data structure

I like the style of this form and would like to use it on my form. I'm using Appnitro Machform, and would like to apply this styling to it. as an owner of a cat boarding kennel; I would like to manage my customers & bookings in an online database. I currently use MACHFORM from APPNITRO for booking-requests; which works great; but this only generates an email-request; I then have to manually create a booking in my offline application "PETLINX" (no online features), export the booking, email a quote, wait for advance-payment, check my banc-account, & approve the booking when all is good. Hello, On our website we use Appnitro (machform) for our forms. We currently have an account set up with Callfire, Callfire has an api we can use, I need someone who can set up the api on the machform php hooks to have the form post into the project we have set up in callfire to have it called right away in callfire. I need this done to where if in the future I need to add it to anotehr form I create, it is easy for me to add in. happy bidding.

Need to modify the "Entries View" section of Mach Forms to include a search feature. Need a fuzzy search on all form fields. Also want to modify Form Selection page to automatically expand to see Entries and Edit option...see images attached. Previous work with Mach Forms preferred.

I have a contact form from Appnitro that I need the PHP script adjusted to be able to add a few calculations fields in order for the customer to view subtotals of what he is ordering. Currently, the contact form only reflects grand total. Fields created will be adjusted in the grand total. Some examples can be provided.

Hello, We use appnitro / machform on our website. Right now I have a form that posts into our lead management system. I have created a new form (just like the one we already did) and I want that posting into our lead management system. Bascially all I need done is the posting code from the form that is already working, copy that code over to the new form and just change the name of the source. That way we can see the different leads posting in. Pretty simple and should be quick.

Currently I use a PHP/MySQL application to create some 6 different forms and collect data (appnitro machform). I am looking for someone who can develop a web based application that will be used to process the submitted data. In the new application an admin should be able to create a few accounts and assign forms to user(s) that can process it's data. These accounts should list and review the submitted forms (display the questions and submitted answers) and add 5 fields (text box and a few checkboxes) with comments to each form. I need an expert in CSS to take the style sheet provided in an add-on PHP Form (MachForm from appnitro) and customize it to match the current look of forms on my website. There are two different forms with 2 different CSS files in MachForm. One needs to fit within the parameters of a small div box. The other form can span the width of the website.
