Hi Freelancers,

I need you to design a little flyer for my hospital 74x105mm.

This flyer will be used from our client to write down their future appointment in our hopital. (See attached flyer of another hospital to get a better idea of what I need)

The flyer should include the following:

Front :

Votre prochain rendez-vous: (your next appointment:)

Include a notice board with the day of the week, date and the time.

Also inlude a remarque field under it.

And a text field under it to write down the conditions

Back:

Company logo and contact info.

To get a better idea of what i clearly need, please check to little flyer of another hospital attached.

About the design, please use blue color, please check our attached business card and logo to see our design.
