I am looking for a flyer (front and back page, 7.5" wide x 10" high) to be designed, using a combination of existing materials and new design. The content can be found in the attached Word Document. Here are additional instructions for what is to be included on each page:

FRONT PAGE:
At the top of this page, use the graphic from the attached PSD file (ZIP). Below this graphic, you will design an "As Featured In" section and include the 4 logos that are attached. Below this, you will design a section that will incorporate the text as found in the 1st and 2nd page of the Word Document. Signature file is attached.

SECOND PAGE:
Your design for page 2 will incorporate the text that is found in the 3rd and 4th page of the Word Document. Note where it shows "[ ]", these are to be some sort of check boxes so the user can select one of the options when they return the form. At the end of the second page you will include the bottom graphic in the attached PSD file, or something similar.

May the best design win! I will be providing feedback throughout the contest.
The winning entry is to provide me with the 2 pages as an editable PSD file(s) upon contest handover.
